Please read these Terms of Service carefully before using Greeley Ride Service. By submitting any form, calling us, or using our services, you agree to these terms.

1. Acceptance of Terms

These Terms of Service ("Terms") constitute a legally binding agreement between you ("you" or "user") and Greeley Ride Service ("GRS," "we," "us," or "our"), a transportation coordination service based in Greeley, Colorado.

By accessing or using our website at GreeleyRideService.com, submitting any form, calling or texting our phone number, or otherwise engaging with our services, you acknowledge that you have read, understood, and agree to be bound by these Terms and our Privacy Policy, which is incorporated herein by reference.

If you do not agree to these Terms, do not use our website or services.

These Terms apply to all visitors, users, riders, caregivers, clinic coordinators, and anyone else who interacts with Greeley Ride Service in any capacity.

2. Who We Are — Important Limitations

GREELEY RIDE SERVICE IS A TRANSPORTATION COORDINATION SERVICE ONLY. WE ARE NOT A HEALTHCARE PROVIDER, LICENSED INSURANCE BROKER, BENEFITS COUNSELOR, MEDICAID ADMINISTRATOR, MEDICARE ADMINISTRATOR, EMERGENCY TRANSPORTATION SERVICE, OR GOVERNMENT AGENCY.

Greeley Ride Service helps Weld County residents explore available transportation options for medical appointments and recurring ride needs. We coordinate rides based on availability, help riders understand what questions to ask about their health plan transportation benefits, and provide educational transportation resources.

We do not:

  • Provide medical advice, diagnosis, or treatment
  • Approve or deny insurance claims or coverage
  • Verify Medicaid or Medicare eligibility
  • Guarantee transportation availability at any time
  • Operate as emergency medical transportation
  • Act as an agent, representative, or affiliate of any insurance company, government agency, or healthcare provider
  • Have any affiliation with Medicaid, Medicare, Health First Colorado, IntelliRide, MediDrive, or any government transportation program

In a medical emergency, call 911 immediately. Greeley Ride Service does not provide emergency transportation services and should not be contacted for medical emergencies.

3. Our Services

Greeley Ride Service provides the following services, all subject to availability, scheduling, service area, and eligibility:

  • Transportation coordination: We coordinate non-emergency medical transportation rides for riders in Weld County and surrounding communities to medical appointments, dialysis treatments, physical therapy, and other healthcare-related destinations
  • Accessible ride coordination: We coordinate rides for riders with mobility needs including wheelchair users, walker users, and others requiring accessibility assistance, subject to vehicle availability
  • Transportation education: We provide free guides, resources, and plain-language information to help riders and caregivers understand available transportation options and potential insurance benefits
  • Ride coordination assistance: We help riders navigate available transportation programs including Medicaid transportation benefits, as an independent service provider with no affiliation with any government program
  • Bilingual support: We provide services in English and Spanish

All services are provided subject to driver and vehicle availability, geographic service area, and scheduling constraints. We reserve the right to modify, suspend, or discontinue any service at any time without notice.

4. No Guarantee of Transportation

SUBMITTING A FORM, CALLING US, DOWNLOADING A GUIDE, JOINING A WAITLIST, OR ANY OTHER INTERACTION WITH GREELEY RIDE SERVICE DOES NOT CONSTITUTE A CONFIRMED RIDE. A RIDE IS NOT CONFIRMED UNTIL YOU RECEIVE EXPLICIT WRITTEN OR VERBAL CONFIRMATION FROM A GREELEY RIDE SERVICE TEAM MEMBER.

Transportation availability depends on multiple factors including but not limited to:

  • Driver and vehicle availability
  • Geographic service area at the time of the request
  • Scheduling constraints and existing ride commitments
  • Weather, road conditions, and other operational factors
  • Insurance or program eligibility verification

We make every effort to provide reliable, consistent transportation service. However, we cannot guarantee that a ride will always be available for every request. We strongly encourage riders who depend on transportation for medical treatments — particularly dialysis patients — to maintain backup transportation arrangements.

Greeley Ride Service is not liable for missed medical appointments, missed treatments, or any health consequences resulting from a failure to provide or confirm transportation.

Ride confirmations are communicated by phone call, text message, or email from a Greeley Ride Service team member. Do not assume a ride is confirmed based solely on form submission or automated acknowledgment messages.

5. No Medical Advice or Coverage Determination

Nothing on our website, in our guides, in our text messages, in our emails, or in any communication from Greeley Ride Service constitutes medical advice, medical recommendations, insurance advice, or coverage determinations.

When we say things like "your health plan may include transportation benefits" or "some riders may qualify for covered rides," these are general educational statements only. They are not a determination that you personally qualify for any benefit, nor a guarantee that any ride will be covered by any insurance plan.

Always verify your insurance coverage, Medicaid eligibility, or transportation benefit status directly with your health plan provider. Greeley Ride Service cannot and does not verify insurance coverage on your behalf.

If you have questions about your health, medical treatment, insurance coverage, or Medicaid eligibility, please consult a qualified healthcare provider, licensed insurance professional, or the appropriate government agency.

6. Communications Consent

By submitting any form on GreeleyRideService.com, calling or texting our phone number, or otherwise providing your contact information to Greeley Ride Service, you expressly consent to receive communications from us including:

  • Text messages (SMS) regarding your transportation requests, ride confirmations, reminders, and follow-up
  • Text messages (SMS) containing promotional content, service updates, and offers
  • Email communications regarding your transportation requests and service information
  • Email newsletters and educational content about transportation resources in Weld County
  • Phone calls from Greeley Ride Service team members

Message frequency varies. Message and data rates may apply. You may opt out of SMS messages at any time by replying STOP to any text message. You may opt out of emails by clicking the unsubscribe link in any email.

This consent is not a condition of receiving transportation services. However, opting out of all communications may limit our ability to confirm rides, send reminders, and provide timely service updates.

By providing a phone number, you represent that you are the account holder or have authorization from the account holder to provide consent for that number to receive text messages.

7. Your Responsibilities

By using Greeley Ride Service, you agree to:

  • Provide accurate, complete, and current information when submitting forms or communicating with us
  • Notify us as soon as possible if you need to cancel or reschedule a confirmed ride — we request at least 24 hours notice when possible
  • Be ready at your confirmed pickup location at your confirmed pickup time
  • Treat our drivers, team members, and other riders with respect and dignity
  • Not use our services for any unlawful purpose
  • Not provide false information about your transportation needs, location, or insurance status
  • Understand that our services are for non-emergency transportation only — in a medical emergency, call 911
  • Maintain backup transportation arrangements for critical medical appointments, particularly recurring treatments like dialysis
  • Verify any insurance benefit information with your health plan directly — do not rely solely on our educational materials for coverage determinations

Greeley Ride Service reserves the right to refuse or discontinue service to any individual who violates these responsibilities, engages in abusive behavior, provides false information, or otherwise misuses our services.

8. Giveaway and Promotions

From time to time, Greeley Ride Service may offer promotional giveaways, complimentary rides, or other promotions. All such promotions are subject to their own official rules, which are posted at GreeleyRideService.com/giveaway-rules.

Key terms applicable to all GRS promotions:

  • No purchase is necessary to enter or win any GRS giveaway
  • Promotions are open only to eligible Weld County area residents as specified in the official rules
  • Complimentary or free rides are subject to availability and must be scheduled within the timeframe specified
  • Promotional rides have no cash value and are non-transferable
  • GRS promotions are not affiliated with, sponsored by, or endorsed by Facebook, Instagram, Meta, or any other social media platform
  • GRS promotions are not affiliated with any government agency, Medicaid program, or Medicare program
  • GRS reserves the right to cancel, suspend, or modify any promotion at any time

9. Intellectual Property

All content on GreeleyRideService.com — including text, graphics, logos, images, guides, forms, and other materials — is the property of Greeley Ride Service and is protected by applicable copyright, trademark, and other intellectual property laws.

You may not reproduce, distribute, modify, create derivative works from, publicly display, or otherwise exploit any content from our website without our express written permission.

You are permitted to download our free transportation guides for your personal, non-commercial use only. Sharing these guides with others for informational purposes is permitted provided you do not modify them and do not remove any Greeley Ride Service branding or attribution.

10. Disclaimer of Warranties

OUR WEBSITE AND SERVICES ARE PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT.

We do not warrant that:

  • Our website will be uninterrupted, error-free, or free of viruses or other harmful components
  • The information on our website is accurate, complete, current, or reliable
  • Transportation will be available for any particular request
  • Any health plan or insurance program will cover transportation costs
  • Our guides or educational content are legally accurate or applicable to your specific situation
  • Any third-party service we reference or link to will be available or accurate

The educational content on our website regarding Medicaid, Medicare, Health First Colorado, and other transportation benefit programs is provided for general informational purposes only and may not reflect the most current program rules, eligibility requirements, or benefit structures. Always verify information directly with the relevant program or agency.

11. Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, GREELEY RIDE SERVICE SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, PUNITIVE, OR EXEMPLARY DAMAGES ARISING OUT OF OR RELATED TO YOUR USE OF OUR SERVICES OR WEBSITE, INCLUDING BUT NOT LIMITED TO DAMAGES FOR MISSED MEDICAL APPOINTMENTS, MISSED TREATMENTS, HEALTH CONSEQUENCES, LOSS OF INCOME, OR ANY OTHER LOSS, EVEN IF WE HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.

Our total liability to you for any claims arising out of or related to these Terms or our services shall not exceed the total amount you paid to Greeley Ride Service in the three (3) months preceding the claim, or one hundred dollars ($100.00), whichever is greater.

This limitation applies regardless of the theory of liability — whether contract, tort, negligence, strict liability, or otherwise.

Some jurisdictions do not allow the exclusion or limitation of certain damages, so some of the above limitations may not apply to you.

12. Indemnification

You agree to indemnify, defend, and hold harmless Greeley Ride Service, its owner, employees, agents, and representatives from and against any and all claims, damages, losses, liabilities, costs, and expenses (including reasonable attorneys' fees) arising out of or related to:

  • Your use of our website or services
  • Your violation of these Terms
  • Your violation of any applicable law or regulation
  • Any false or inaccurate information you provide to us
  • Your conduct in connection with our services

13. Governing Law and Disputes

These Terms shall be governed by and construed in accordance with the laws of the State of Colorado, without regard to its conflict of law provisions.

Any dispute arising out of or related to these Terms or our services shall first be submitted to good-faith informal negotiation. Please contact us using the information in Section 15 to initiate this process. We will make every reasonable effort to resolve disputes informally.

If informal resolution is unsuccessful, disputes shall be resolved in the state or federal courts located in Weld County, Colorado. You consent to the personal jurisdiction of such courts.

Nothing in this section prevents either party from seeking injunctive or other equitable relief in any court of competent jurisdiction for matters involving intellectual property rights or confidential information.

14. Changes to These Terms

We reserve the right to modify these Terms at any time. When we make material changes, we will update the "Last Updated" date at the top of this page and post the updated Terms at this URL.

Your continued use of our website or services after the effective date of any changes constitutes your acceptance of the updated Terms. We encourage you to review these Terms periodically.

If you do not agree to the updated Terms, you must stop using our services and notify us that you wish to discontinue your relationship with Greeley Ride Service.
